What we do

This check is to assess your risk of developing heart disease, type 2 diabetes and stroke. It also has the potential to reduce your risk of developing these conditions.

This check is part of a national scheme to prevent the onset of these health problems. By having this check and following the advice of your health professional you could improve your chances of living a healthier life.

The Cwm Taf Health Check team invite eligible people to attend for their health check in GP practices throughout Cwm Taf. All eligible people in Cwm Taf will receive an invite.

Who is it for?

Everyone between the ages of 40-74, who has not been diagnosed with cardiovascular disease, type 2 diabetes, stroke or high blood pressure will be invited for a health check.

If you are outside the age range and concerned about your health you should contact your GP.

Can anyone use this service?

Those eligible will receive an invite by letter or telephone

Opening Times

9am -5pm some earlier/later appointments may be available

 

What to expect

The check will take 30-40 minutes.

You will be asked about your lifestyle, family history and choices which may impact on your health.

Your weight, height, blood pressure, age, sex and ethnicity will be recorded.
A finger prick blood sample will be used to test your blood sugar and cholesterol level.

Your risk of developing vascular disease in the next 10 years will be discussed and you will be given personalised advice on how to lower your risk and maintain a healthy lifestyle.

You may be offered a referral to local services for support.
