What we do
Our Valley LIFE project is about making sure people with dementia, their carers and families receive timely support to improve their wellbeing and quality of life.
A key part of this work includes developing two new health and wellbeing centres for people who have cognitive and memory problems, in particular dementia. The first of our Health and Wellbeing Centres is now in place on the Ysbyty George Thomas site, while plans are progressing for another centre in Merthyr Tydfil.
We will be working with individuals, carers and family members, voluntary organisations, and our clinicians and social workers so we can provide:
- early diagnosis, tailored treatment programmes and on-going support for people with dementia;
- a range of services for people with a dementia which helps to support people to stay well at home for longer;
- a range of additional support services in people’s communities, such as extra-care housing options and respite facilities.
Who is it for?
The services under the Valley LIFE project, in particular the Health and Wellbeing Centres, are for anyone with dementia and their families and carers. We also hope the wider community will get involved with the centres.
Can anyone use this service?
The Health and Wellbeing Centres will be available to those with dementia, their families and the community
What to expect
The new facility promotes a move away from hospital based care and places the focus on how people with dementia can live well within the community if they have the right level of support in place.
A range of services including day care, assessments, clinics, community nursing, therapies and care home teams as well as local authority and third sector services, will be delivered from the Centre, which has been designed to reflect the strong history of the local area.
