What we do
Physiotherapists will visit you at home if you are housebound. This may be due to a chronic/ longterm condition or serious illness.
We provide assessment, diagnosis and treatment to help improve function and independence.
This will be delivered in your own home by physiotherapists and assistants.

Who is it for?
The service is for adults over the age of 18 who are unable to leave their homes to undergo treatment elsewhere (Housebound), and who are residents of Merthyr Tydfil or Rhondda Cynon Taf.
Can anyone use this service?
We initially need a referral from a professional for Community Physiotherapy. However, Reablement services can be accessed by contacting your local authority.

What to expect
A member of the physiotherapy team will carry out a detailed assessment of your condition and how it affects your daily life.
We will test a number of movements as well as your strength and/or balance, to see how well you manage at home.
After your assessment, we can talk about what you’d like to work on. You might be given exercises to complete at home and we may visit you again or refer you to other services.
