What we do
The primary aim of the service is to facilitate self-management and living well with neurological conditions.
The Community Neuro-Rehabilitation Service supports people with neurological conditions to help them improve and maintain their emotional and physical health, and to develop and achieve their goals.
Service users will have the opportunity to take part in activities such as memory workshops, fatigue and pain management strategies, community projects, and other activities while getting to know other people with similar difficulties.
The service works closely with voluntary organisations and social services to share knowledge and resources.
Who is it for?
This service is for individuals who have been clinically diagnosed by a health professional as having a neurological condition (excluding Dementia), such as epilepsy, Multiple Sclerosis, Motor Neurone Disease, Parkinson’s disease, Guillain–Barré Syndrome, Acquired Brain Injury, stroke, tumours or other infective or genetic neurological conditions.
The service is for adults aged 18 years and older who live in Merthyr Tydfil or Rhondda Cynon Taf. Those referred must have a willingness and ability to engage in groups/projects in their area and/or brief individual intervention.
Families and carers of service users can also access the service for support.
Can anyone use this service?
Individuals require a referral from an allied health professional or a consultant to access the service.
Monday to Friday 8:30am – 4:30pm (excluding Bank Holidays)
What to expect
Once we receive your referral, you will be contacted by a member of our team who will ask you some basic questions in order to understand the kind of support you might benefit from.
Depending on your circumstances, we might follow this up with a more detailed assessment. This gives you an opportunity to let us know more about your situation and what groups you might be interested in attending.
